One membership. Your plan, built around you.
Your membership is the foundation of your care at Tulsa Longevity Center. It covers your ongoing relationship with your provider. Then you add only the treatments your plan calls for, so you pay for what you use and nothing you don’t.
Core Membership $149/month
Every member gets:
Ongoing care with Holly O’Hearn, APRN-CNP, who builds and adjusts your plan
A 45–60 minute initial visit with Holly to review your history, goals and labs and build your plan
A 30-minute monthly visit with Holly, in person or by telehealth, whichever is most convenient for you
A full review of your lab results, with a plain-language explanation of what’s in range, what’s worth watching and a personalized treatment plan
Thyroid and gut health review as part of your plan
Secure messaging with our care team between visits

Priority scheduling
Need to be seen before your next monthly visit? Additional and same-day visits are $75 for members.
Telehealth visits are available when you’re located in Oklahoma at the time of the visit.
About labs: your membership covers the review and interpretation of your results. The lab draw and testing are billed separately, either through your insurance or at our cash rate. We’ll help you choose the option that costs you less.

Tirzepatide weight loss $299-$399 /month (dose dependent)
Semaglutide weight loss $299 /month
Women’s hormone therapy $75 /month
Testosterone therapy for men $120 /month

Advanced Peptide Therapy $129 /month per therapy

How memberships work
Book a free call. We’ll answer your questions and explain your options.
Foundation Consult and labs. Your provider reviews your history and labs and recommends the add-ons that fit you. The $250 consult fee is waived when you join.
Start your plan. Your membership and add-ons are billed together, once a month.
Ongoing care. Regular visits and labs keep your plan current, and you can add or drop add-ons as your goals change.
Terms
Core memberships are a 12-month commitment, billed monthly.
Your add-ons can change month to month as your plan changes.
After your first year, your membership continues month to month and you can cancel anytime with 30 days’ notice.
Some treatments, including pellet insertions and certain controlled medications, require an in-person visit.
If you cancel before 12 months, a one-time charge of two months of membership ($298) applies. This charge doesn’t apply if you move out of Oklahoma or if your provider ends your care for medical reasons.
All therapies are prescribed only after an evaluation and only when medically appropriate. Membership contents vary based on your provider’s recommendations. Some therapies are controlled substances. Compounded medications are prepared by a licensed pharmacy for individual patients and are not FDA-approved. Individual results vary.
